Notice of Insurance Practices and of Patient Financial Responsibility 

General Policies

 Appointments.  When scheduling an appointment, please provide your name, date of birth, phone number, chief reason for visit, and any updated contact information. Please be on time. 

Appointment Notice and Cancellation Fee:  SoulPath Integrative Health, PLLC requires at least 72 hours advance notice prior to cancellation of any scheduled appointment.  If you miss your scheduled appointment, or cancel with less than the above advance notice, SoulPath Integrative Health, PLLC will charge a cancellation fee of $200 or 50% of the consultation fee to your credit card, whichever is greater.  100% of the appointment fee for subsequent late cancellations after 72 hours will be charged.

 Additional Authorization for Credit Reports:  I hereby authorize SoulPath Integrative Health, PLLC permission to obtain personal credit reports on me to ascertain my financial responsibility regarding payment for services rendered. 

 Return of Products: All supplements ordered through FullScript may be returned according to their Return Policy. Lab kits may be returned according to Rupa Health’s Return Policy.

 No Refunds.  SoulPath Integrative Health, PLLC does not offer any refund for office visits or services of any kind.

 Online and phone communication; Electronic signatures. Dr Ana Luisa Neal stores medical and office records digitally. While we make reasonable efforts to keep the data secure according to legal requirements, and maintain the privacy and confidentiality of patient data, no system is 100% secure. We consider electronic signatures as the legal equivalent of manual signatures on all signed Agreements, and manifest consent to be legally bound by this Agreement’s terms and conditions.

Office Insurance Practices and Patient Financial Responsibility

 Payment: SoulPath Integrative Health, PLLC accepts credit cards and PayPal.  SoulPath Integrative Health, PLLC bills to my debit or credit card on file unless you provide alternate payment information and instructions.

 No Participation in Insurance Plans: SoulPath Integrative Health, PLLC is an out-of-network provider for services within this practice; SoulPath Integrative Health, PLLC does not participate in any insurance panels, and does not accept assignment from any insurance company. Consequently, all patients/clients are responsible for payment in full at time of service or within a mutually agreed upon payment plan and charges are determined by SoulPath Integrative Health, PLLC.

 No Responsibility To Determine Eligibility for Benefits: SoulPath Integrative Health, PLLC is not responsible for determining eligibility for benefits or for assisting with collecting insurance benefits and has no responsibility to correspond with or telephone or email any insurer with which SoulPath Integrative Health, PLLC is an out-of-network provider.

  My Financial Responsibility: SoulPath Integrative Health, PLLC may provide you with an itemized statement (or “super-bill”) to present to your insurance carrier.  However, you are financially responsible for any charges for services even should your insurer determine that those services are non-covered or are unreasonable, medically unnecessary or inappropriate. You also agree to be responsible for costs and expenses, including court costs, attorney fees and interest, should it be necessary for SoulPath Integrative Health, PLLC to take action to secure payment of an outstanding balance. 

 Medicare: SoulPath Integrative Health, PLLC does NOT accept Medicare or Medicaid patients in this practice.  The patient is responsible for all charges.

Charges: In addition to the consultation fee, SoulPath Integrative Health, PLLC may also charge me for specific services such as blood or urine tests and blood drawing and handling. SoulPath Integrative Health, PLLC often recommends testing through a specialty lab for information that cannot be tested through a conventional lab. This is one of the ways to dig more deeply into what may be causing or contributing to symptoms.  I understand that SoulPath Integrative Health, PLLC cannot guarantee that recommended lab testing (through a conventional lab or specialty lab) will be covered. It is your responsibility to check with your insurance company prior to getting any recommended lab work done to check on coverage.
